The Hieline Take
The Optima is the back to look at when a sling back has stopped holding you upright. The aluminum shell gives a firm base, the cut-out at the bottom leaves room for the pelvis to sit back where it should, and the low pivot point means changing the angle does not drag the pelvis with it. The standard Optima has a mild 3" contour for users who move a lot and want their shoulders free. The DEEP version wraps further around the trunk and its laterals set from 5.5" to 7", which suits users who need more help staying centered. The laterals on both bend in or out by hand, so the fit can be tuned after it is on the chair. Pick the width of your back canes, choose a height that supports you without getting in the way of your arms, and add laterals or a head support only if you need them. Check the weight limit for your width before you order, because it rises from 300 lbs. to 400 lbs. at 21" wide.

A back support sits against the body, so once it ships it cannot be returned. Measure your back canes before you order and use the build steps above. The warranty is unaffected, so a defect is still covered.

How do I choose the width?
Measure between the outside of your back canes. Each Optima fits chairs from its own width up to 2" wider, so a 16" back fits 16" to 18" chairs. NXT makes widths from 13" to 29".
How do I choose the height?
NXT makes 7", 9", 13", 16" and 19" shells, though not every height at every width. The Back height step only offers the heights NXT makes at the width you picked. A taller back gives more trunk support; a shorter one leaves more room for the arms when you push. NXT's product sheet starts its height range at 9"; its order form lists 7" backs from 13" to 20" wide, and we follow the order form.
Optima or Optima DEEP?
The Optima has a mild 3" contour for active users who want free movement. The Optima DEEP has deeper laterals that adjust from 5.5" to 7" for more midline support. Both come in the same sizes at the same price.
What is the weight limit?
300 lbs. up to 20" wide and 400 lbs. at 21" and wider, from NXT's user manual. With the 4-point fixed hardware, backs 23" and wider are rated 550 lbs. When the back is on a wheelchair, the lower of the two limits applies.
Why do 22" and wider backs cost more?
NXT lists backs 22" to 29" wide at a higher price, billed under HCPCS E2621 instead of E2620. They cost $42.40 more here.
Which mounting hardware should I choose?
The standard QuickFit quick release hardware lifts off with one hand and adjusts while you sit. If you never take the back off, NXT swaps in fixed hardware at no charge. The 2-point fixed gives 1" of depth and 25 degrees of angle and fits backs 16" tall or lower. The micro mount is the lightest and fits backs 13" tall or lower. The 4-point fixed is the most stable, with 1" of depth and about 15 degrees of angle. The micro mount and the 4-point fit one chair width only. NXT's product sheet names the 4-point kit NIB-1307; its order form names it NIB-1969, and we order it from the form.
Does a 13" back have the one-hand release?
No. NXT says the one-hand release does not fit backs narrower than 14". A 13" back still comes with the QuickFit quick release hardware.
What do I need for a head support?
A pad, the head support hardware and the receiver bracket. NXT requires all three, and the page will not add a pad without the other two. The flip back kit attaches to the NIB-0747 hardware only.
How do the lateral trunk supports work?
Each lateral is one side, with its arm and pad. Choose left and right on their own, so you can fit one or a pair. NXT ships the swing away laterals set up for the right side, and the instructions in the package turn one for the left.
What is the Infection Control cover?
A fluid proof cover that wipes down, made by NXT to the back model you order. NXT lists it at $205 for backs, and it is $164.00 here. The note on NXT's cushion page calls the Infection Control cover a no charge option; its back support list prices this one, and we follow the back support list.
How do I clean it?
Machine wash the cover warm (140°F) on a gentle cycle with regular detergent. Do not bleach, iron or dry clean it. NXT recommends inspecting the back monthly, washing the cover and shell monthly or as needed, and checking the fasteners every 6 months.
What does the warranty cover?
NXT warrants the back support for 24 months from delivery to the first user against defects in materials and workmanship. It is repaired or replaced if the surface does not stay intact under normal wear within 18 months. Cosmetic wear, misuse, improper installation, unauthorized changes, cigarette burns, tears from sharp objects and damage from improper washing are not covered.
Can I return it?
No. A back support sits against the body, so once it ships it cannot be returned. Measure your back canes before you order. The warranty is unaffected.
